Supports and Services

Personal Care
If needed, we can support you to maintain your health and well-being by assisting you to complete a range of tasks as part of your daily routine.

Home Care
If needed, we can support and assist you to complete a range of tasks around your home and in your community so you can stay independent, active and connected.

Home Delivered Meals
At times when you are unable to prepare regular meals, we can deliver meals to your home. Meals are healthy, appetising and culturally appropriate.

Staying Active and Connected

Older and Active In Darebin
Older and Active in Darebin is a booklet that lists a range of community organisations and programs that are run by local older people, or welcome older people as participants. For a copy of the booklet contact 8470 8828 or download a copy here.

Community Transport
If you need support with transport for shopping, attending social groups or other activities in the local area the Darebin Community Bus can pick you up. This is a pre-booked door to door service.

Darebin Bus
Darebin Bus is a 9 seater mini bus available for hire by Incorporated not for profit community groups servicing our local community.

Support for Carers

Respite Care
This service gives carers a chance to have a break from their caring responsibilities. Respite can be provided in the home or in the community.

Additional Information

Yanada House
Yanada House is a dementia specific Day Centre, which offers support to people with memory loss and their carers. Yanada offers a program of social and recreational activities, which are planned to meet individual needs and abilities. For more information refer to the brochure or contact 9481 9570.

Home Care Packages
Darebin City Council provides a limited number of Home Care Packages to people who have been assessed as eligible. Home Care Packages provide a coordinated package of services to meet specific needs. For more information refer to the brochure or contact 8470 8828.

Aboriginal Residents
If you are an Aboriginal resident and would like to talk to someone about the services that promote independence to be active and connected in Darebin contact 8470 8828.

This will be discussed with you at our first visit to your home. A list of fees is available and updated annually.

Personal information provided by you or your referee is kept confidential according to Darebin City Council’s Privacy Policy.

What is the process?

Contact us: You can contact us yourself or have a family member, friend, doctor, or health service contact us with your consent.

Assessment: An assessment officer will contact you to make a time to visit you at home. To receive services you will need an assessment. We will talk with you about:
- What you can do for yourself.
- What you need help with to stay safe and well at home.
- What you want to be able to do or keep doing.
- What activities are important to you outside the home.

Plan: We will then make a plan with you which lists what you want to achieve and the supports and services available to assist you.
